Dear family member/resident representative,
I am writing to provide you with information about the new Stop and Watch form we have introduced at your loved one’s aged care centre.
Stop and Watch is an early warning communication tool that assists in identifying changes in a resident’s daily routine. The form is designed to provide an effective communication tool for visitors, volunteers and contractors to identify signs of deterioration early and escalate concerns so staff can respond quickly to a resident becoming unwell.

When should the form be used?
You are encouraged to use the Stop and Watch form any time your loved one is ‘not their usual self’ or you identify a change in their usual condition. We will also be encouraging other visitors such as allied health contractors and volunteers to utilise the form.
How do I use the form?
Copies of the Stop and Watch form will be available from reception at your loved one’s aged care centre.
If you notice when visiting something different in a resident or their daily routine, circle the change(s) as outlined on the Stop and Watch form, write the name of the resident along with your name and hand the form to an administration staff member at the care centre or a care team member who is looking after your loved one.
From here, the member of staff will provide the form to the Registered Nurse In Charge who will complete a clinical review, determine next steps and devise a plan of care in collaboration with your loved one, their resident representative, and the resident’s GP where required, as established from the findings of the review. Your loved one and their resident representative will then be informed of the details of the review and the plan of care.
