Skin Care and Wound Management Policy and Procedure

Dear family member/resident representative,

I am writing to let you know that following a comprehensive review, IRT has updated its Skin Care and Wound Management Policy and Procedure.

The review took place to ensure we provide consistent, high quality care to your loved one while complying with best practice in relation to wound and skin care.

We have updated our approach to skin care and wound management to emphasise the importance of collaborative and holistic wound and skin assessment and care tailored to the individual resident, as well as the importance of preventative measures to maintain a resident’s skin integrity.

Our updated approach includes a new and improved Wound/Skin Management Plan and Evaluation (wound management chart) in our electronic records system (which will go live in the coming weeks) to improve documentation requirements in regards to wound and skin care.

Our staff are receiving training on the updated policy and procedure content this month.

If you have any questions regarding the new policy and procedure or you wish to discuss your loved one’s skin and wound care needs, please speak with the Care Manager or the Registered Nurse In Charge.
