How it works

Getting started is simple, and we’re here to support you every step of the way.

Apply for an assessment

Contact My Aged Care on 1800 200 422 to check your eligibility  and book a free in-home assessment. An assessor will identify the services that best match your needs.

Confirm your funding

You’ll receive a written confirmation outlining your approved care level, the services you’re entitled to, the government’s contribution, and any gap fee you may need to pay.

Start receiving care

Choose an approved provider within 56 days of your confirmation. Together, you’ll design a care plan around your goals, lifestyle, and budget, then your services begin.

Benefits & Eligibility

Clinical Care may be right for you if:

You’re living with a long-term or complex health condition

You need help managing medications, wounds or treatments

You’re recovering from a hospital stay, illness or surgery

You want to stay living at home while receiving ongoing medical support

You could be eligible if:

You’re aged 65+ (or 50+ for Aboriginal and Torres Strait Islander people)

You live independently and want to stay at home longer

You’ve been assessed by My Aged Care

You have health needs that require professional care at home

What will I pay?

Clinical Care is fully funded by the Australian Government for eligible older Australians.

If you receive funding through My Aged Care, there is no out-of-pocket cost for Clinical Care services such as nursing, wound care, chronic disease support and allied health.

That means you can receive professional health care in your home without worrying about fees or unexpected bills.

As each type of service is funded a little differently, we’ll make sure you understand what to expect before services begin.

Clinical Care provides professional health support in your home, including nursing, medication support, wound care and allied health services. It’s designed to help you manage your health, recover from illness and stay well while continuing to live at home.

No, a referral isn’t required, although your GP may recommend services. Eligibility is confirmed through My Aged Care.

Yes. Your services are planned around your routine and preferences wherever possible.

We aim to provide consistent care so you can build relationships with familiar faces.

Yes. Clinical Care can be provided alongside Independence Support and Everyday Living services as part of one personalised plan.

If you’re eligible for the Support at Home Program, government funding may help cover the cost of your services.

Your care plan is reviewed regularly and can be adjusted if your needs change over time.

Switching is easy! Just:

  • Give our friendly customer service team a call on 134 478
  • Let My Aged Care know and ask for your referral code to be reactivated
  • Inform your current home care provider
  • Start getting home care services from IRT when your previous arrangement ends.

You’ll be issued with a final statement from your old provider and your unspent funds will be transferred to your new account with IRT.
