Home Care Services
The End-of-Life Pathway provides additional, short-term support to help you remain at home with comfort and dignity if you or a loved one have been assessed as having three months or less to live.
Many older people wish to remain in their own home, surrounded by familiarity and family, as they near the end of life. The End-of-Life Pathway provides additional funding and support to help make this possible for up to 12 weeks, with the option to extend to 16 weeks if funding remains. You may be eligible if a medical practitioner or nurse practitioner confirms you have three months or less to live.
To access the pathway, you will need a high-priority aged care assessment through My Aged Care. This can be triggered by:
Your representative requesting an assessment and providing the form to the assessor
Contacting My Aged Care directly on 1800 200 422
Participants on the End-of-Life Pathway receive $25,000 for home care services over 12 weeks. If funding remains at the end of this period, services can continue for up to 16 weeks total.
You may be asked to contribute to independence and everyday living services, but clinical supports (such as nursing) are fully funded by the fovernment and require no contribution.

A medical practitioner or nurse practitioner completes the End-of-Life Pathway form. This information is used by My Aged Care to arrange a high-priority aged care assessment to confirm eligibility.
Once approved, you’ll work with us to develop a personalised care plan that reflects your needs, preferences and wishes for remaining at home.
You’ll receive coordinated home care services for 12 weeks, with the ability to continue up to 16 weeks if funding is still available.
At the end of the program, we’ll review your progress with you and help plan any ongoing support you may need.
