Home Care Services
The Restorative Care Pathway offers short-term, focused support after an illness or injury, helping you stay independent and confident at home.
If you’re eligible, you can add up to 16 weeks of restorative care to your current Support at Home services. You can receive coordinated clinical services such as nursing and allied health to help build strength and confidence at home. With the right support, the Restorative Care Pathway helps you:
Stay independent for longer and delay the need for higher levels of care
Regain confidence with daily activities after illness or injury
Manage new or changing age-related conditions
Learn practical skills to maintain strength as you age
You’ll receive a budget of around $6,000 for your 16-week restorative care program. We can also apply for an additional $6,000 if you need extra support.
You may be asked to contribute to independence and everyday living services, but clinical supports such as nursing are fully funded, with no contribution required.
If you need a little helping hand to keep living at home and need home care, the first step (unless you’re paying privately) is to register with My Aged Care and get an assessment. This is the Australian Government’s aged care services portal where you can access information and apply for funding.
You’ll need a My Aged Care assessment to access the Restorative Care Pathway. During this assessment, the assessor will discuss your goals, what’s becoming harder at home and whether restorative care could help you stay independent.
If you’re eligible, we’ll work with you to set your goals and develop a personalised restorative care plan outlining the support and therapies you’ll receive.
Your home care team will deliver focused, short-term support to help you build strength, confidence and independence at home towards your care.
At the end of the program, we’ll review your progress with you and help plan any ongoing support you may need.
