Our residential care neighbourhood, Vitality, offers general care for residents with similar care needs. It caters for residents who are mentally and physically active or require minimal assistance to move around their environment or minimal prompting to aid cognition.

Vitality focuses on maximising resident wellbeing and creating a supportive environment where people come to live, not just receive care. Our holistic approach ensures that medical, physiological, emotional, social and relationship needs are catered for.

Our model of care

IRT’s Journey of Care is a specialised model of care that ensures residents age in the right place and with the right care within our aged care centres. We recognise that every journey is unique and honour the diversity of each resident’s care needs by personalising the experience for every resident. 

A key component of this specialised model are neighbourhoods: defined needs-based residential areas within our aged care centres that cater for residents with similar care needs. This allows for staff specialisation and tailored design of the physical environment and equipment in each neighbourhood, ensuring families see their loved one ageing in the right place with the right care.

We currently have three Journey of Care neighbourhoods:

  • Vitality – Offers residential (or more general) care for residents who require less assistance
  • Serenity – Offers nurturing palliative care for residents approaching end of life
  • Discovery – Caters for residents with dementia and utilises dementia design principles

How to apply for an ACAT assessment and residential aged care

Step 1

Get an ACAT assessment

Find out whether your loved one is eligible for government-funded services using My Aged Care.

You can enquire online or call 1800 200 422. You may need a member of ACAT to visit your loved one at home to assess their eligibility.

Step 2

Talk to our aged care team

Submit an enquiry below or call 134 478, and one of our friendly  team members will call you to discuss you and your loved one’s needs and preferences.

They’ll help you request an Income and Assets Test with My Aged Care, if you need one

Step 3

Confirm your booking

Once the Care Manager confirms your place at your preferred IRT aged care centre, we will be in contact with you to confirm arrangements with you and your loved one.

If you’ve been approved for government funding support, it’s paid to us on your behalf.
